Prescription & Optician Details
In order to purchase Contact Lenses online, we need the details of a valid prescription and the contact details of your optician. If in doubt, rest assured we will contact your optician to confirm these details on your behalf.
We also require you to send us your contact lens prescription. It must be signed, dated, have the name, address and contact number of your optician/contact lens practitioner.
Please visit your own optician to obtain a valid contact lens prescription. To make it easier, you can print the form below and ask your optician to complete.

When we receive the prescription we will process your order immediately.
Please note: To comply with UK regulations, we need your Optician's details. When placing an order, you grant us permission to contact your optician to verify your prescription details, if required.
